Case Manager – Dreams’ Cuisine Workforce Dev.

Hourly Pay: $21.00 - $23.00

The Case Manager is responsible for providing advocacy, information and referrals for participants within the Dreams’ Cuisine Workforce Development report. This is a full-time position providing case management services to trainees’ participating in the program.

Responsibilities

  • Conduct intake and assessments for new participants
  • Develop service plan with a focus on employment barrier removals, financial stability and housing stabilization
  • Provide individualize resources and referrals to community programs
  • Assist with clients in advocating for services they qualify for
  • Provide classroom training and on-job-training support
  • Work with partner Housing Specialists for client re-housing
  • Develop partnerships with other community based organizations
  • Track outcomes and data management of participants’ activities and accomplishments
  • Follow-up with participants who have exited the program
  • Assist in oversight of interns during program operations
  • Other duties as assigned

Skills

The incumbent must demonstrate the following skills:

  • Bachelor’s Degree in Social Services, Human Services, or similar degree, or related field experience required. 
  • Must have a minimum of 2-3 years in program services
  • Demonstrates good judgment and has the ability to work independently and as a part of a team
  • Be driven to succeed, organized, proactive, innovative, responsible, assertive, and flexible
  • Possess excellent written and verbal communication and interpersonal skills
  • Have the capacity to effectively and efficiently handle multiple tasks and deadlines simultaneously
  • Must be able to work collaboratively with staff, program partners and volunteers
  • Must possess a solid understanding of data collection
