Dreams for Change is seeking a dedicated and qualified Housing Navigator to support our County-funded Safe Parking Program in El Cajon. This position will serve individuals experiencing homelessness by providing supportive services that include advocacy, resource referrals, and housing navigation support.
RESPONSIBILITIES
- Set up and break down program operations
- Conduct intake and assessments for new participants
- Develop service plan with a focus on financial stability and re-housing
- Create individualized housing plan with path to permanent housing
- Support individuals to build skills and knowledge to secure appropriate housing
- Collaborate with housing providers and build relationships with landlords
- Assist participants in building skills to maintain housing once its secured
- Provide individualized resources and referrals to community programs
- Assist clients in advocating for services for which they are eligible
- Provide employment development support
- Develop partnerships with other community based organizations
- Outcomes and data management into HMIS for participants activities and accomplishments
- Follow-up with participants who have exited the program
- Assist in oversight of interns during program operations
- Meet for supervision bi-weekly
- Other duties as assigned
