Case Manager – Safe Sleeping

Hourly Pay: $21.00 - $24.00 plus benefits

The Case Manager is responsible for providing advocacy, information and referrals for participants within the DFC homeless service programs of either Safe Parking or Safe Sleeping. Multiple positions available with both first and second shift. Part-time positions are second shift.

Responsibilities

  1. Set up and break down program operations
  2. Conduct intake and assessments for new participants
  3. Develop service plan with a focus on financial stability and re-housing
  4. Provide individualized resources and referrals to community programs
  5. Assist with clients in advocating for services for which they qualify
  6. Provide employment development support
  7. Work with homeless service team for client re-housing
  8. Develop partnerships with other community-based organizations
  9. Outcomes and data management into MHIS for participants activities and accomplishments
  10. Follow-up with participants who have exited the program
  11. Meet bi-weekly with supervisor for case review
  12. Other duties as assigned

Qualifications

  • Bachelor’s Degree in Social Services, Human Services, Public Health, or similar degree, or related field experience is required. 
  • Must have a minimum of 2-4 years in program services
  • Demonstrates good judgment and has the ability to work independently and as a part of a team
  • Be driven to succeed, organized, proactive, innovative, responsible, assertive, and flexible
  • Possess excellent written and verbal communication and interpersonal skills
  • Must be able to work collaboratively with staff, program partners and volunteers
  • Must possess a solid understanding of data collection and program evaluation
  • Effective negotiation and mediation skills
  • Effective written communication skills
  • Computer skills including the ability to work with spreadsheets, word-processing, and databases at a highly proficient level

 Peer Support Assistant – Safe Parking Oceanside

Hourly Pay: $19.00

Dreams for Change (DFC) is seeking qualified candidates with lived experience to create client-centered recreational, life skills, and wellness programming at the Safe Parking Program funded by the City of Oceanside. The goal of the Peer Support Assistant is to build community through various activities while reducing stress or isolation. The Peer Support will be able to share their own experience including successes and challenges. The Peer Support will provide feedback received from clients and will work with the Safe Parking Program team to enhance services at the Safe Parking Program. This is a part-time position at 20 hours per week.
