Case Manager

Salary: $22.00/hr.

The Case Manager is responsible for providing advocacy, information, and referrals for participants within the Safe Sleeping Program. The Case Manager works in collaboration with referring outreach workers to support clients. This is a full- time position at 40 hours per week.

Responsibilities

  • Set up and break down program operations
  • Set up and break down program operations
  • Conduct intake and assessments for new participants
  • Develop service plan with a focus on financial stability and re-housing
  • Provide individualized resources and referrals to community programs
  • Assist clients in advocating for services for which they qualify
  • Provide employment development support
  • Work on a re-housing plan with client
  • Develop partnerships with other community based organizations
  • Outcomes and data management into HMIS for participant activities and accomplishments
  • Follow-up with participants who have exited the program
  • Assist in oversight of interns during program operations
  • Meet for supervision bi-weekly
  • Other duties as assigned

Lived Experience Ambassador

Hourly Pay: $17.00

Dreams for Change (DFC) is seeking qualified candidates with lived experience to create client centered recreational, life skills, and wellness programming at the Safe Sleeping Program. The goal of the Lived Experience Ambassador is to build community through various activities while reducing stress or isolation. The Lived Experience Ambassador will be able to share their own experience including successes and challenges. The Lived Experience Ambassador will provide feedback received from clients and will work with the Safe Sleeping Program team to enhance services at the Safe Sleeping Program. This is a part-time position at 20 hours per week.
