Housing Navigator

Housing Navigator
The Housing Navigator is responsible for providing advocacy, information and referrals, and housing navigation assistance for participants within the Safe Parking Program. This is a full-time position, with partial hours on site at safe parking from 5:30p.m.-9:30p.m. five evenings a week and additional hours off site for a total of 40 hours.
Responsibilities
●Develop service plan with a focus on financial stability and re-housing
●Create individualized housing plan with path to permanent housing
●Support individuals to build skills and knowledge to secure appropriate housing
●Collaborate with housing providers and build relationships with landlords and property management companies
●Assist in searching for housing opportunities for clients
●Develop a list of property management companies and landlords with affordable units
●Assist participants in building skills to maintain housing once its secured
●Provide individualized resources and referrals to community programs
●Assist clients in advocating for services for which they are eligible
●Develop partnerships with other community-based organizations
●Outcomes and data management into HMIS for participants activities and accomplishments
●Follow-up with participants who have exited the program
●Assist in oversight of interns during program operations
●Meet for supervision bi-weekly
●Other duties as assigned

Program Coordinator
Dreams for Change is looking for an experience program coordinator for the Safe Parking Program. The Program Coordinator is responsible for developing, planning, organizing, implementing, and coordinating a variety of activities. The Coordinator will work collaboratively with the Program Manager, and partnering agencies to ensure project goals are met and accomplished. The Coordinator is responsible for daily program operations.
Responsibilities
●General oversight of program operations, staff, interns and volunteers
●Return potential client’s messages
●Assign clients to a program site
●Track each site’s enrollment numbers and manage wait list when necessary
●Attend community meetings
●Develop community partners to support client needs
●Facilitate case consultation meeting with program staff and interns
●Collect and report outcomes and demographic data
●Coordinate meal services and donations ensuring distributions to each site
●Order supplies for sites
Qualifications
● Preferred Bachelor’s Degree in Social Services, Human Services, Nutritional Services, or similar degree, or related field experience required.
● Must have a minimum of 3 years in program services
● Demonstrates good judgment and has the ability to work independently and as a part of a team
● Be driven to succeed, organized, proactive, innovative, responsible, assertive, and flexible
● Possess excellent written and verbal communication and interpersonal skills
● Have the capacity to effectively and efficiently handle multiple tasks and deadlines simultaneously● Must be able to work collaboratively with staff, program partners and volunteers
● Must possess a solid understanding of data collection and program evaluation
● Must possess a valid CA driver’s license with clean driving record

The Case Manager is responsible for providing advocacy, information, and referrals for participants within the Safe Sleeping Program. The Case Manager works in collaboration with referring outreach workers to support clients. This is a full- time position at 40 hours per week.

●Conduct intake and assessments for new participants
●Develop service plan with a focus on financial stability and re-housing
●Provide individualized resources and referrals to community programs
●Assist clients in advocating for services for which they qualify
●Provide employment development support
●Work on a re-housing plan with client
●Develop partnerships with other community based organizations
●Outcomes and data management into HMIS for participant activities and accomplishments
●Follow-up with participants who have exited the program
●Assist in oversight of interns during program operations
●Meet for supervision bi-weekly

Lived Experience Ambassador

Lived Experience Ambassador
Dreams for Change (DFC) is seeking qualified candidates with lived experience to create client centered recreational, life skills, and wellness programming at the Safe Sleeping Program. The goal of the Lived Experience Ambassador is to build community through various activities while reducing stress or isolation. The Lived Experience Ambassador will be able to share their own experience including successes and challenges. The Lived Experience Ambassador will provide feedback received from clients and will work with the Safe Sleeping Program team to enhance services at the Safe Sleeping Program. This is a part-time position at 20 hours per week.
Responsibilities
●Design client-centered recreational activities
● Connect with outside speakers as appropriate to lead various groups and activities
● Listen to client concerns and feedback
● Share own experience as a way to support clients
● Work with Program Manager and Case Managers to address client concerns
● Complete notes and reporting as needed
● Follow and enforce program policies
● Other duties as assigned
